Do I need to be a current patient to participate in the program?
No, new patients are more than welcome to follow guidelines and participate in the whitening program.

What is required of me to participate in the program?
In order for you to participate in the program, you must be at least 18 years of age and have had a complete dental examination with recommended x-rays. You must stay current in keeping with recommended dental health appointments and make all scheduled appointments. If you need to cancel an appointment, that should be done with a minimum 48 hour notice and rescheduled within the next 2weeks.

Can I participate in the program if I have an outstanding dental bill?
No, you may not participate in the program if you have any outstanding bills. Once your account is up to date and have met all other requirements, you may then participate in the program.

Will the teeth whitening procedure be performed in the office?
The actual procedure will not be performed in the office. The dental office will construct custom/professional trays for all eligible participants. The patient will be scheduled for custom impressions of their teeth, and whitening will be ready for pick up within 48 hours. A whitening syringe will be given to patients along with an instruction pamphlet to complete the whitening procedure at home.

How will I know how to use my free whitening kit?
Your whitening kit will include an instruction pamphlet which will guide you on how to use the free whitening kit at home.

How often will I have to visit with the dentist to participate in the program?
In order to participate in the program, you will need to stay up to date with dental appointment visits every 6 months.

Am I eligible for a new whitening tray if mine is lost or destroyed?
New trays can be purchased at a cost to the patient.

What happens if I need to cancel a previously scheduled appointment?
If you need to cancel a scheduled appointment for whatever reason, it should be done within 48 hours of the appointment. In order to remain a participant in the program, you must reschedule the appointment within 2 weeks.

Can I purchase additional whitening gel syringes if I remain faithful to the program?
The initial whitening gel syringe is complimentary and you may purchase additional syringes.

What happens if I fail to comply with program guidelines?
Failure to comply with program guidelines will end in termination of whitening program.

Will I have the opportunity to be reinstated into the program if I become terminated?
Yes, if you become terminated from the program you will be eligible to be reinstated after 12 months of loyalty.

What happens if I have a negative reaction to the at-home whitening kit?
If you experience a negative reaction to the at-home whitening kit, it is recommended to discontinue use and consult a physician or dentist immediately.

Am I eligible for participation in the program if I have previously canceled appointments?
Yes, as long as you meet all other requirements, you remain eligible to participate in the program regardless of previously cancelled appointments.

Can I participate in the program if I am loyally paying installments of previously completed dental work?
Yes, if you are paying monthly installments of previously completed dental work, you are eligible to participate in the whitening program. If you fail to continue monthly payments, you will be disqualified from the program.
