Whitening For Life – Terms & Conditions

For all existing and new patients who keep their scheduled appointments (a minimum of two additional hygiene visits per year) following their first hygiene visit and all recommended dental care is treated thereafter, custom made trays will be made and they will receive a free take home whitening kit. If patients continue to keep all of their appointments that are scheduled every six months, touch up whitening kits will be provided free of cost after their hygiene visit.

Dental Office Responsibilities:

  • Oakwood Dental Arts will construct custom trays for all patients participating in the program.
  • We will provide patient with one complimentary whitening gel syringe at their scheduled hygiene appointment following their initial hygiene visit and after all recommended dental care is treated.
  • Our office will provide patients with detailed instructions on how to properly use the at-home whitening kit and to answer all questions or concerns the patient may have.
  • Lost of destroyed trays can be replaced at a cost to the patient.
  • If any patient should fail to meet requirements, they will be disqualified from the program. Once patient has successfully met all requirements for twelve months, they will have the opportunity to be reinstated into the program.

Patient Responsibilities:



  • Patients must be at least 18 years of age in order to participate in this program, unless dental or overall health conditions do not allow it. Patient cannot be pregnant or nursing.
  • Patient is required to have had a complete dental examination with recommended x-rays.
  • Patient must be staying current in keeping with recommended dental health appointments as well as, adhere with the minimum required dental care as treatment recommended by the dentist.
  • Once all dental care is completed, patient will have impressions taken for professional whitening system and custom trays will be constructed.
  • Patients must attend all scheduled appointments. If a cancellation needs to be made, patient should do so with a minimum 24 hour notice and original appointment must be rescheduled within two weeks.
  • In order to receive any promotional discounts, patient account must be current* (no outstanding patient balance).
  • If you are an existing patient who has already had whitening treatment performed in our office, you are automatically enrolled in the program.

*This does not include patient accounts repaying monthly installments on time of previously completed dental work.

Lifetime Maintenance Rules & Regulations:

  • Patient must maintain minimum continued care as treatment planned and appointed by Oakwood Dental Arts, its dentists, and staff.
  • Patient must maintain continued care with a hygiene appointment every six months.
  • Patient must abide by all policies regarding payment and broken appointments.
  • ONE whitening solution refill will be given to patient at each re-care appointment. (A maximum of TWO whitening gel tubes annually).
  • Lost of destroyed applicator trays can be replaced at a cost to the patient.

Offer Terms and Conditions:

  • This offer may not be combined with any dental insurance benefits.
  • Oakwood Dental Arts reserves the right to refuse on-going whitening treated if deemed necessary based on the patients health conditions, misuse, abuse, or any other factor deemed necessary to void the offer.
  • If custom trays become lost or destroyed, patients may purchase a replacement for $___.
  • Patients who lose eligibility will need to successfully meet all requirements for a minimum or twelve months to be reinstated into teeth whitening program.
  • New patient exams must be completed by a general dentist and not a specialist.
